Pharmacist in Primary Care Services

BPC has recruited and employed a team of experienced Community Pharmacists to work within Primary Care as part of an NHS England scheme. The Clinical Pharmacist work acrosss a number of our Member Practices to support Practices and their patients.

This programme is carried out via a mixture of hub working and place based in practice work. This provided an environment for our team to work as a unit on projects collabratively and embedded them with practice teams.

The Clincial Pharmacists carry out a variety of work including processing medication changes post hospital discharge, medication reviews, support medication optimisation work and face to face clinics with patients on a range of issues including Asthma, COPD, INR, AF.

The Care Quality Commission (CQC) inspected the Extended Hours Primary Care Service in February 2018 and rated us as "Good".
